passengers and cargo, type and registration marks of each aircraft, and call sign of each aircraft radio station;
c)Frequency of service and schedule thereof (indicating them by a diagram);
d)Outline of facilities for maintenance and for operational control.
2. The following exhibits shall be attached to the application under the preceding paragraph:
(1)Copy of license issued by the foreign State to which the applicant's nationality belongs, for air transportation service on the said air routes;
(2)Where applicant is a juridical person, the articles of corporation;
(3)The latest profit and loss Statement and balance sheet; and
(4)Conditions of transport.
(Application for Approval of F&es, Rates and Charges)
Article 233. Any person who will obtain approval for the establishment or alteration of fares, rates and charges referred to in Article 129-(2)of the Law shall submit to the Minister of Transport an Application for approval of establishment (alteration)of fares, rates and charges in describing the following items at least thirty days prior to the proposed date of introduction:
(1)Name, address and nationality;
(2)Amount of fares, rates and charges to be established or to be altered and basis of calculation (in case of alteration, indicating comparison of the present and the proposed);
(3)Effective date proposed; and
(4)In case of alteration of fares, rates and charges, reasons thereof
(Application for Approval of Alteration of Business Program
Article 233-(2). Any person who will obtain approval for alteration of his business program under Article 129-(3)paragraph 2 of the Law shall submit to the Minister of Transport an application describing the following items at least forty-five days prior to the proposed date of operation in the case of alteration in ftequency or when any type of aircraft in use is to be altered to one considerably different in its loading capacity, at least thirty days prior to the proposed date of operation in the ease of alteration other than above;
(1)Name, address and nationality;
(2)Matters which will be altered (indicating difference between the present and the proposed);
(3)Effective date proposed; and
(4)Reasons necessitating the alteration.
